For the 2026/2027 season, we're creating something truly special.

Our brand-new BHF FC home shirt will feature the names of loved ones who are sadly no longer with us, permanently embedded into the fabric of the shirt. Every time our players step onto the pitch, they'll be carrying the memories of those who meant so much to us all.

Whether it's a parent, grandparent, partner, sibling, child, relative or close friend, this is an opportunity to honour their memory while supporting a cause that continues to save lives.

Every name tells a story. Every name represents someone who is loved, remembered and never forgotten. Together, these names will create a unique shirt that symbolises remembrance, hope and the incredible community behind BHF FC.

Supporting the British Heart Foundation

A donation is requested for every name submitted, with all proceeds helping us continue to raise vital funds for the British Heart Foundation.

Heart and circulatory diseases affect millions of families across the UK. By adding your loved one's name to our shirt, you'll not only keep their memory alive but also help fund life-saving research and support for families facing heart disease.

Simply complete the form below and submit the name of your loved one.

• Please check the spelling carefully, as names will appear exactly as submitted.
